As a vendor and serial journaler, I created this LADS-themed vendor form to help me stay organized. I love making lists and reflecting on my experiences, and this form lets me do both! At the end of each event, I add it to my journal — not only does it keep me organized, but it also becomes a little keepsake to look back on.

This printable is free for everyone to use. We kindly ask that:
